The “scream” in not made by the engine, but by the inner gun port in each wing. In the outer two ports in each wing, the gun muzzles are flush with the port, but the inner gun is set back about four inches, laving a cavity in the port. The airflow resonates in this cavity, producing the scream.
I learned this from a video featuring the P51 “Quick Silver”, in which the owner, Scott Yoak, explained the P51 scream.
